The Elder Care program manager manages a team of assigned members and provides ongoing care coordi! nation and outreach. The Elder Care Department has an interdisciplinary care team (ICT) that includes a geriatrician, social worker, registered nurses, a nurse practitioner, clinical pharmacy specialist (CPS), and a licensed practical nurse. The ICT focuses on the care of the frail and elderly members, with primary focus on Special Needs Plan (SNP) Medicare members. In collaboration with the inpatient case management team, primary care provider (PCP) and Hospitalist team, the Elder Care registered nurse provides support, follow-up and assists in development of a care plan for eligible members post-hospital discharge.
Essential Functions:
Outreaches to new and existing SNP members annually and within 90 days of enrollment to perform a telephonic assessment of health and psychosocial needs. Maintains records of dates and types of assessments completed. Remains available to assist with coordination of care as needed throughout the year
Participate ! in the weekly IDT meeting and develops plan of care in collabo! ration with the ICT and the PCP; based on outcome and findings make telephonic follow-up calls to coordinate any immediate care needs.
Responsible for managing SNP members transitions pre- and post-hospital discharge to complete a nursing assessment that will drive the development of a care plan, determine intervention(s) needed, assess educational needs and assist in coordination of care for optimal outcomes.
Participate in hospital rounds telephonically to monitor the care of hospitalized SNP members. May also outreach to SNF and LTC facilities to get report on admitted members.
Performs other duties as assigned.
Qualifications
Basic Qualifications:
- Bachelor of Science, Nursing (BSN)
- Licensure as a Registered Nurse in the State of Georgia
- A minimum of 2 years experience in ambulatory/ outpatient care, preferably in gerontology
- Masters! of Science in Nursing
- A minimum of 4 years of experience in geriatric assessment
- Prior experience in self-management education programs
- Geriatric Nursing Certification
